Massage Feedback Forms

Can any help with wording for massage feedback forms. I have a couple of clients that return to me for treatments and who make wonderful comments about their massage treatment but I do not have anything tangible to record their complimentary opinions. I am thinking of making a feedback folder whereby potential and current clients can look at feedback but I'm a little stumped for wording and how many Q's to ask besides the Why did you choose massage therapy? Have you benfitted from your treatments? Would you recommend massage therapy to friends? Is providing a name optional.

For the wonderful comments that your clients say about your treatments you could get a nice book for them to write in this. You can ask if it would be ok for you to include their comments as testimonials on your marketing info and website and for others to look at the comments.

You can still do a form to get client feedback. However I wouldn't want others to see what they said - one for client confidentiallity and also if they said anything negative.

Getting feedback from clients is a good way to find out what you are doing right and what you might be doing wrong. If you post it to them and get them to fill it in anonymously& post it back you may get more honest answers. A question you could ask is - is there anything I could do to improve your treatment? What changes have you noticed since having treatments? You could include a voucher with an incentive such as an extra 15 mins or similar to encourage them to complet the form.
