Hi

I will be starting to do massages very soon. Can any-one please help me with a sample of a consultation form. Will really appreciate this.

Have you qualified yet? This should have been something that was covered in your course.

it has also been covered several times on this forum, suggest you use he search facility,

consultation forms

Dear BGFL and Aromababe

Thanks for your responses. I qualified in 2003 but never had the confidence to practise. The consultation form we used as students was about 4 pages long. On the advice of BGFL I found what I was looking for by searching previous discussions.
