Or are you talking about recording the information for your case study?

The college gave us one which requires you to note down stuff like lifestyle, health, contraindications etc, but also has whether you had to amend the massage in any way and a self review section on how you thought it went.

If it is this sort of form I transferred it into Excel and I can certainly e-mail you one if you want it. (Blank of course!!:D)
